Service Agreement

All clients and customers shall agree to the following prior to the first appointment.
This agreement then shall remain in effect for all future appointments until revoked in writing
by either party within 24 hours prior to the next-coming appointment.

This Service Agreement / Hold Harmless Agreement (“Agreement”) is made and entered into on __________ [date], by and between Lindsay McNutt, Concierge Canine Care (“Caregiver”) and ______________________________ [pet owner’s name] (“Owner”).

Service(s) to be Provided 

Caregiver agrees to provide the following services for Owner’s pet(s):

Future appointment services shall be arranged separately via the best communication method agreed upon between Owner and Caregiver with confirmation in writing.

 Payment Terms 

Owner agrees to pay for the service(s) provided by Caregiver according to the current rate schedule. Payment is due upon completion of services.  For multi-day visits, 50% deposit is due 24 hours prior to the first scheduled visit with the remaining balance due within 24 hours after final scheduled service.  Deposit is fully-refundable with a 24-hour written cancellation notice prior to the first scheduled appointment.

Pet Owner’s Representations 

Owner represents that their pet(s) is/are in good health and has received all necessary vaccinations, including Rabies, DHLPP, and Bordetela. Owner confirms that their pet(s) is/are free from any contagious diseases and has not shown aggressive behavior towards other pets or people in the past twelve (12) months.

Medical Treatment 

Owner authorizes Caregiver to seek emergency veterinary care for their pet if necessary. Owner agrees to be on call in an emergency to provide payment over the phone as needed.  Caregiver is not liable for any injuries or damages caused by or fees due to the veterinary clinic.

Liability Waiver 

Owner agrees to hold harmless and indemnify Caregiver from any claims, actions, liabilities, damages, or expenses arising out of or related to the care of their pet(s), including but not limited to injury, illness, or death of the pet(s).

Personal Property 

Caregiver is not responsible for the loss or damage of any personal property belonging to the Owner, including pet toys, leashes, household items, or other items left with the Caregiver.

Governing Law 

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas.

Amendments 

This Agreement may be amended only by a written agreement signed by both parties.

Signatures 

By signing below, both parties agree to the terms and conditions outlined in this Agreement.
